New Patient Information

Please ensure that you have the following before you call for your appointment. Our Doctor personally reviews each and every new patient’s records before we schedule you so that we can provide you with the most complete care.

When you call you will need:

  1. Names and telephone numbers of the doctors who are treating you
  2. Authorization from your current providers to release your records
  3. Primary care doctor’s contact information (if you have one)
  4. Your current, active insurance information
  5. What medications you are taking
  6. The dosage amount of these medications and how many you regularly take
  7. The contact information of the physician who writes your prescriptions
  8. Any recent MRI, Cat Scan or X-ray reports
  9. The facility name and contact information where your studies were performed (if you have had any recently)

Please review the Forms section and complete the new patient packet and authorizations prior to your initial office visit.

We will make every effort to get you scheduled and in to see the Doctor as soon as possible. We must have all the information above to begin the process; the more prepared we are, the better we can serve you.

Our office strives to provide our patients with the highest quality care. We allow enough time for you to meet with the Doctor for a complete evaluation. Therefore, we require a full 24 hours notice for any cancellations. If proper notice is not given, a $100.00 cancellation fee with be assessed to your account and will be payable by you within 15 days.

We Accept Insurance: Medicare, Private Insurance, HMOs/PPOs, Automobile Insurance and Worker’s Compensation
